Looking for help w/ identifying a story

Hi everyone-
Looking for help with the identification of a short story I've heard about but haven't been able to track down. Don't have the author (and apparently don't have the correct name), but it's something akin to "Notes from an Abandoned Farmhouse". The synopsis I've read says the premise is entries from a journal detailing the last days of a teenage boy trapped in a farmhouse while something hunts/haunts him from the woods outside.
Anyone know this one? Google doesn't find my version of the title, so I'm sort of stuck. Any help at all would be appreciated.
